2996663183-2 Apply for this job now Job Description Hartford Healthcare seeks a Primary Care Nurse Practitioner / Physician Assistant to deliver comprehensive outpatient care in our Healthcare & Medical Services department. In this role, you will provide assessment, diagnosis, and management of acute and chronic conditions, focusing on prevention, wellness, and patient education. You'll collaborate with a multidisciplinary team within a culture rooted in teamwork, compassion, and excellence. Responsibilities Provide comprehensive primary care to adult and adolescent patients, including assessment, diagnosis, and treatment of acute and chronic conditions. Conduct routine physical exams, wellness visits, and preventive screenings in an outpatient setting. Order, interpret, and act on diagnostic tests, labs, and imaging in accordance with evidence-based guidelines. Develop and manage individualized care plans, including medication management and chronic disease follow-up. Collaborate closely with physicians, nurses, and allied health professionals to coordinate patient-centered care. Educate patients and families on diagnoses, treatment options, lifestyle changes, and self-management strategies. Use electronic health records to document visits, update problem lists, and maintain accurate, timely clinical documentation. Participate in quality improvement initiatives, population health efforts, and care coordination programs. Support same-day access and urgent visits within the primary care practice as needed. Adhere to all regulatory, safety, and compliance standards while modeling Hartford Healthcare's values of teamwork, compassion, and excellence. Required Skills Primary care clinical assessment and diagnosis Chronic disease management (e.g., diabetes, hypertension, COPD) Preventive care and wellness counseling Ordering and interpreting labs and diagnostic imaging Electronic Health Record (EHR) documentation (e.g., Epic or similar) Medication management and prescribing within scope Patient education and health coaching Care coordination and interdisciplinary collaboration Urgent/acute care management in outpatient setting Evidence-based practice and clinical guidelines application
