Role:
We are looking for a compassionate Primary and Urgent Care Provider (MD, DO, APRN, NP, FNP, PA) to join our team of highly skilled clinical providers in locations across Connecticut. Responsibilities:
Providing and managing immediate patient care, including physical examinations, evaluations, assessments, diagnoses, and annual well-visits. Engaging with patients and their families or caregivers to understand and monitor self-management care plans. Examining patients, evaluating medical histories, addressing patients' questions and concerns. Ordering, performing, and interpreting diagnostic tests and explaining the results to patients. Documenting individualized care plans and care coordination in the patient chart. Analyzing patients' medical history, medication allergies, physical condition, and examination to determine the best care. Updating patients' charts and information to reflect current findings and treatments. Prescribing pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions. Refer patients to specialists or to internal medicine as appropriate. Directing and coordinating patient care activities of nursing and support staff. Participating in meetings to evaluate practice processes and workflows pertaining to patient care and quality improvement efforts. Participating in health promotion, education, and/or prevention programs, such as PCMH and ACO. Following established policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
Requirements:
Active medical licensure and ability to prescribe medications in CT. Knowledge and experience in diagnosing and treating a wide variety of illnesses. Primary Care and Urgent Care experience is a plus.
Supplemental pay:
Bonus structure available.
Pay:
$120,000.00 - $220,000.00 per year
