Job Description

We are seeking an experienced Advanced Practice Registered Nurse to join established outpatient primary care practices in Southington and Plainville, Connecticut. This is an excellent opportunity for an APRN who enjoys providing comprehensive primary care and building long-term relationships with patients while working within a collaborative, well-supported healthcare environment. The APRN will evaluate, diagnose, treat, and manage a broad range of acute and chronic medical conditions. Responsibilities will include routine physical examinations, preventive care, chronic disease management, medication management, ordering and interpreting diagnostic testing, patient education, referrals, and follow-up care. The ideal candidate will have prior experience in primary care, family medicine, internal medicine, or a similar outpatient setting and should be comfortable practicing with a high degree of clinical independence. Responsibilities Provide comprehensive outpatient primary care Evaluate and treat acute illnesses and chronic medical conditions Perform histories, physical examinations, and preventive care visits Order and interpret laboratory and diagnostic testing Prescribe and manage medications Manage conditions including hypertension, diabetes, hyperlipidemia, asthma, COPD, and other common primary care diagnoses Educate patients regarding prevention, lifestyle modification, and disease management Coordinate referrals and specialty care as needed Maintain accurate and timely documentation Develop strong, ongoing relationships with patients Qualifications Current or eligible Connecticut APRN license Appropriate national board certification Previous primary care, family medicine, internal medicine, or comparable outpatient experience Strong clinical judgment and ability to practice independently Excellent communication and patient-care skills This position offers the opportunity to join an established Central Connecticut healthcare organization with access to a broad network of physicians, specialists, diagnostic services, hospital resources, and ancillary support. $130,000.00 - $150,000.00 per year
