In this role, you will manage chronic and acute conditions, perform comprehensive geriatric assessments, and develop individualized care plans that prioritize independence, safety, and quality of life. Responsibilities Provide comprehensive primary care to geriatric patients, including assessment, diagnosis, and treatment of chronic and acute conditions Develop, implement, and adjust individualized care plans focused on aging-related health needs and patient goals Coordinate care with specialists, nurses, social workers, and other team members across the Hartford Healthcare network Conduct regular wellness, cognitive, and functional status evaluations to promote independence and quality of life Educate patients and families on disease management, medications, prevention, and community resources Document all patient encounters, orders, and care plans accurately in the electronic health record (EHR) Participate in quality improvement initiatives, care pathways, and population health efforts for older adults Collaborate in interdisciplinary team meetings and case reviews to optimize patient outcomes Supervise or mentor junior staff, students, or residents as appropriate Maintain compliance with clinical guidelines, regulatory standards, and Hartford Healthcare policies Required Skills Geriatric medicine Primary care clinical assessment and diagnosis Chronic disease management (e.g., CHF, COPD, diabetes) Medication management and deprescribing Electronic Health Record (EHR) documentation Care planning and care coordination Geriatric functional and cognitive assessment Palliative and end-of-life care principles Patient and family education Interdisciplinary team collaboration
