Job Description

Incredible Health seeks a Nurse Practitioner to deliver compassionate, evidence-based care while supporting our mission to improve nurse staffing and patient outcomes. In this hybrid clinical role, you'll perform assessments, diagnose, prescribe, and coordinate care in collaboration with interdisciplinary teams. You'll leverage EHRs and data insights to drive quality, efficiency, and safety, while contributing clinical expertise that informs our tech-enabled solutions for hospitals and nurses nationwide. Responsibilities Conduct comprehensive assessments, diagnose conditions, and develop evidence-based care plans Provide direct patient care, including prescribing and managing medications per scope of practice Collaborate with physicians, nurses, and care teams to optimize patient outcomes and care transitions Use EHR systems and data insights to document, track, and improve quality of care Educate patients and families on treatment plans, prevention, and chronic disease management Participate in quality improvement initiatives that enhance safety, efficiency, and patient experience Support Incredible Health's mission by sharing clinical expertise that informs tools and resources for clinicians Required Skills Advanced patient assessment and diagnosis Care planning and care coordination Electronic Health Record (EHR) documentation Evidence-based clinical decision-making Prescriptive authority and medication management Patient and family education Interdisciplinary collaboration Clinical triage and prioritization Quality improvement and outcomes tracking Data-informed practice and analytics literacy
