We provide comprehensive primary care, behavioral health, chronic disease management, preventive medicine, telehealth, and in-home medical services to a diverse patient population, with a strong commitment to serving the Hispanic community. Duties Provide comprehensive outpatient care with a primary focus on Chronic Care Management (CCM), Remote Patient Monitoring (RPM), same-day sick visits, and Transitional Care Management (TCM)/hospital discharge follow-up visits . Evaluate, diagnose, and treat acute illnesses during same-day appointments. Conduct post-hospital discharge and emergency department follow-up visits to reduce readmissions and ensure continuity of care. Manage a panel of patients enrolled in CCM by reviewing chronic conditions, medications, treatment plans, and coordinating ongoing care. Review and interpret Remote Patient Monitoring (RPM) data, identify abnormal trends, and intervene when clinically appropriate. Develop, implement, and update individualized care plans for patients with multiple chronic conditions. Perform medication reconciliation following hospital discharge and during chronic care visits. Order and interpret laboratory tests, diagnostic studies, and imaging as appropriate. Prescribe medications and adjust treatment plans within scope of practice. Collaborate with physicians, specialists, home health agencies, pharmacies, and other healthcare providers to coordinate patient care. Educate patients and caregivers on chronic disease management, medication adherence, lifestyle modifications, and preventive care. Accurately document all patient encounters and ensure compliance with Medicare, Medicaid, CCM, and RPM billing requirements. Work closely with care coordinators, medical assistants, and the clinical team to optimize patient outcomes and quality metrics. Maintain high-quality, compassionate, patient-centered care while meeting productivity and documentation standards. Participate in quality improvement initiatives and value-based care programs. Skills Active and unrestricted Florida Physician Assistant or Nurse Practitioner license. Current DEA registration and BLS certification (or ability to obtain prior to start date). Minimum of 1 year of primary care, family medicine, internal medicine, urgent care, or hospital medicine experience preferred. Experience with Chronic Care Management, Remote Patient Monitoring, Transitional Care Management , and value-based care is strongly preferred. Strong clinical assessment, diagnostic, and treatment planning skills. Ability to independently manage both acute and chronic medical conditions. Knowledge of Medicare and Medicaid documentation and billing requirements for CCM, RPM, and TCM preferred. Excellent communication and patient education skills. Strong organizational and time-management abilities with the ability to manage multiple patient encounters efficiently. Experience using Electronic Medical Records (EMR/EHR) systems. Ability to work collaboratively with physicians, care coordinators, medical assistants, behavioral health providers, and other members of the healthcare team.
