Job Description

Overview We are seeking a dedicated and compassionate Autonomous Nurse Practitioner to join our mobile healthcare team 3-5 days a week. This position involves in-home visits and video visits. The ideal candidate will provide high-quality patient care across various settings. This role requires a strong commitment to patient wellness, effective communication skills, and the ability to work collaboratively within a multidisciplinary team. Duties Conduct comprehensive assessments of patients, including physical exams and health histories. Develop and implement individualized care plans based on patient needs. Manage patients with chronic conditions. Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times. Educate patients and families on health management, disease prevention, and wellness strategies. Participate in utilization management to optimize patient outcomes and resource allocation. Adhere to infection control protocols to ensure a safe environment for patients and staff. Collaborate with other healthcare professionals to coordinate comprehensive care. Experience Candidates should possess the following skills: Strong background in patient care, particularly in wound care and critical care settings. Proficiency in medication administration and knowledge of infection control practices. Familiarity with behavioral health issues and treatment modalities. We are looking for individuals who are passionate about improving patient outcomes while maintaining the highest standards of professionalism in all interactions.
