Responsibilities Perform comprehensive patient histories, physical exams, and risk assessments. Diagnose and manage common acute and chronic conditions using evidence-based guidelines. Prescribe medications and treatments, order and interpret diagnostic tests, and adjust care plans as needed. Provide preventive care, health education, and counseling to support better patient outcomes. Document visits accurately in the EHR and ensure compliance with clinical, legal, and regulatory standards. Collaborate with physicians, pharmacists, nurses, and care managers to coordinate holistic, patient-centered care. Triage patient concerns, manage same-day visits, and escalate complex cases appropriately. Participate in quality improvement, population health initiatives, and performance metrics.
