Job Description

Weekly onsite rounding at a skilled nursing facility, rather than a traditional clinic-based pain-management position. We are seeking a dedicated and experienced Nurse Practitioner/Physician Assistant with an interest or background in Pain Management to provide onsite care at a skilled nursing facility. The Provider will conduct weekly pain management rounds, evaluating residents with acute and chronic pain conditions and working closely with the facility's nursing team to ensure appropriate, safe, and effective pain management. Responsibilities will include assessing residents, reviewing current pain medications and treatment plans, making appropriate medication adjustments or recommendations, documenting visits, and providing ongoing follow-up during weekly rounds. This position is ideal for an NP/PA looking for a consistent, flexible weekly schedule with the ability to develop ongoing relationships with residents and the facility's clinical team. Responsibilities Conduct onsite weekly pain management rounds at the skilled nursing facility Evaluate residents experiencing acute or chronic pain Review current medications, pain-control regimens, and treatment response Develop and update individualized pain-management treatment plans Prescribe and manage medications when clinically appropriate and within scope of practice Monitor medication effectiveness, side effects, and potential drug interactions Consider non-opioid and non-pharmacologic treatment options when appropriate Coordinate care with nursing staff, primary care providers, specialists, therapy, and other members of the interdisciplinary team Provide follow-up evaluations during subsequent weekly rounds Complete timely and accurate clinical documentation Qualifications Active Nurse Practitioner or Physician Assistant license in the state of practice Active NPI number Current DEA registration if prescribing controlled substances is required Ability to prescribe medications Experience in pain management, geriatrics, skilled nursing, primary care, or a related specialty preferred Strong clinical judgment and communication skills Comfortable managing medically complex and elderly patients Reliable and able to maintain a consistent weekly rounding schedule Schedule One onsite visit per week at the assigned skilled nursing facility. Scheduling can be coordinated with the facility and is designed to provide flexibility for the provider. This is an excellent opportunity for a Nurse Practitioner seeking flexible, recurring clinical work while providing an important service to residents of a skilled nursing facility.
Pay:
$150,000.00 - $250,000.00 per year
