09/08/2026 Job Description Job Description Under direction of Medical Director and/or collaborating physician will provide health care services in any agency clinic as assigned. This includes, but not limited to the diagnosis and treatment of STDs and other acute and chronic health problems; performing routine health exams and lab testing. Provide education to clients and staff on prevention and healthcare issues. Follow standing orders, policies and procedures of agency along with IDPH and CDC programmatic rules and regulations. The APN will refer and/or consult with collaborating physician in all health care service beyond the level of his/her expertise.
Required Skills, Education, & Experience:
Licensed in Illinois as a Certified Family Nurse Practitioner in good standing; Maintains Liability Insurance coverage; Maintains and promotes the advancement of collaborative and professional medical and nursing practice; Ability to maintain satisfactory working relationships with other employees and clients Capable of working with individuals and groups to promote good health and disease prevention; Requires thorough knowledge, skills and experience working with and utilizing community resources; Candidates must own a dependable automobile and possess a valid driver's license and auto insurance; Will accept any other duties as assigned by ESHD; Drug Free Environment; Drug Screening may be required.
