LivWell Community Health Services is a non-profit organization providing comprehensive health and support services, education, and advocacy, as related to HIV/AIDS, sexually transmitted diseases, and viral hepatitis. Quality health care is provided to persons living with HIV/AIDS or sexually transmitted diseases, support services are provided to HIV clients and their families, and prevention education and testing are provided to all clients, their families, and the community at large. Within the community, we strive to prevent new infections of HIV and STI's and reduce the stigma associated with these various diseases by promoting awareness, acceptance, and equality.
Position Overview:
The Nurse Practitioner at Hopkinsville, KY site reports directly to the Site Director and is responsible for providing comprehensive primary and specialty healthcare services to patients at LivWell Community Health Services. The Nurse Practitioner manages a broad range of clinical responsibilities, including patient assessment, diagnosis, treatment planning, medication management, preventive care, chronic disease management, and health education, with a focus on individuals living with HIV/AIDS and those at risk for HIV infection. In collaboration with physicians, nurses, medical staff, case managers, behavioral health professionals, pharmacists, and community partners, the Nurse Practitioner delivers patient-centered, culturally competent, and evidence-based care. The Nurse Practitioner will support organizational quality improvement initiatives, promote positive patient outcomes, and contribute to enhancing the overall health and quality of life of the communities served.
Duties/Responsibilities:
Conduct comprehensive patient assessments, including medical histories, physical examinations, and diagnostic evaluations. Provide comprehensive primary care services including health assessments, diagnosis, treatment, and management of acute and chronic illnesses. Diagnose and treat acute and chronic medical conditions in accordance with evidencebased clinical guidelines Provide HIV prevention, testing, treatment, and ongoing management of patients living with
HIV/AIDS
Prescribe medications and treatments in accordance with state regulations and organizational policies Monitor patient progress and adjust treatment plans as appropriate Deliver preventive health services, including vaccinations, screenings, and wellness counseling Educate patients and families regarding disease management, medication adherence, risk reduction, and healthy lifestyle practices Collaborate with interdisciplinary team members to coordinate comprehensive care services Order, interpret, and evaluate laboratory tests, diagnostic studies, and imaging results, and other treatments within the scope of practice and state regulations Participate in quality improvement initiatives and population health management activities Develop and implement individualized treatment plans Provide preventive care services including annual wellness exams, screenings, immunizations, and health education Perform minor office procedures within scope of practice Manage chronic conditions such as diabetes, hypertension, asthma, HIV, COPD, and behavioral healthrelated conditions Maintain accurate, timely, and complete documentation within the EHR Ensure compliance with all federal, state, and local regulations, including Ryan White Program requirements, HIPAA, HRSA standards, and organizational policies. Participate in community outreach, education, and health promotion activities to expand access to care Promote continuity of care and reduce barriers to healthcare access Serve as a point of contact for providers regarding questions on patients' care, history, and other matters that may be timeconsuming or require special attention Work closely with physicians, nurse practitioners, physician assistants and other healthcare providers to coordinate patient care, improve workflows, and support the clinic All other duties as needed
Required Skills/Abilities:
Strong understanding of clinic operations, patient care coordination, healthcare regulations, and compliance standards Strong interpersonal and communication skills with the ability to effectively interact with patients, staff, providers, and leadership Experience with electronic health record (EHR) systems and Microsoft Office applications Knowledge of HIPAA regulations, OSHA standards, and healthcare accreditation requirements Commitment to LivWell's mission of providing compassionate, equitable care Cultural sensitivity and commitment to reducing healthcare disparities Ability to build trusting relationships with diverse patient populations Strong organizational and time management skills Ability to maintain confidentiality and professionalism in all interactions
Qualifications:
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program Licensure as a Nurse Practitioner for the state of Kentucky Current unrestricted Nurse Practitioner license in the state of Kentucky National board certification as a Family Nurse Practitioner (FNP) or related specialty. Current DEA registration and prescriptive authority. Willingness to receive HIV certification within 18 months of hire BLS certification required, ACLS preferred Experience in primary care, infectious disease, HIV/AIDS care, public health, or underserved populations preferred. Strong clinical assessment, communication, and patient education skills Willing to occasionally travel for outreach events, training, and community engagement Ability to stand, walk, sit and use clinical equipment for extended periods and lift up to 25 pounds Ability to work in a fastpaced healthcare environment
