Seeking a Family Nurse Practitioner (FNP) or Outpatient Primary Care Practice to Expand Behavioral Health Integration Services in Somerset, Kentucky Role Description The Family Nurse Practitioner is a full-time, on- site role based in Somerset, KY. This role provides patient-centered primary and behavioral healthcare services, including comprehensive assessments, diagnosis, and evidence-based treatment planning for individuals and families. Day-to-day responsibilities include conducting in-person visits, managing acute and chronic conditions, prescribing and monitoring medications, ordering and interpreting diagnostic tests, and coordinating care with therapists, counselors, and psychiatrists. The practitioner will educate patients and families on health promotion, mental wellness, and disease prevention, while documenting care accurately and maintaining compliance with regulatory and organizational standards. Collaboration with the multidisciplinary team and active participation in quality improvement initiatives are central to this position. Demonstrated clinical expertise as a Family Nurse Practitioner, with a strong foundation in Nursing and Medicine. Proficiency in providing Primary Care services, including management of chronic and preventive health needs. Ability to deliver urgent and acute care, including experience in Urgent Care or similar fast-paced clinical settings. Active, unencumbered Nurse Practitioner license in Kentucky, with national board certification as a Family Nurse Practitioner (FNP). Experience or strong interest in behavioral health and integrated care within community or outpatient settings. Excellent clinical judgment, communication, documentation, and interpersonal skills for working with diverse populations. Commitment to trauma-informed, culturally responsive, and person-centered care in rural and underserved communities. Graduate degree from an accredited Nurse Practitioner program; current BLS certification required (ACLS preferred).
