- Full-Time , Days Boulder Community Health is seeking a compassionate and collaborative Nurse Practitioner or Physician Assista nt to join our Family Medical Associates team.

- Graduation from a Physician Assistant or Nurse Practitioner program as required by licensure.
- Current Colorado Physician Assistant or Nurse Practitioner License.
- Must be credentialed by the Boulder Community Hospital Medical Staff Office prior to hire date.
- BLS certification required by the end of conditional period.
- ACLS certification required within 6 months of hire. Responsibilities
- Provide same-day, next-day, primary, and acute care services for patients across the lifespan.
- Obtain medical histories, perform physical examinations, assess, diagnose, and develop treatment plans.
- Order, interpret, and manage laboratory tests, diagnostic studies, medications, immunizations, and minor procedures within scope of practice.
- Collaborate with physicians and the care team to manage acute and chronic medical conditions.
- Educate patients and families on treatment plans, medications, preventive care, and available community resources.
- Respond to patient calls, provide clinical consultation, and support continuity of care between visits.
- Evaluate and manage urgent and emergent patient care needs.
- Maintain accurate and timely clinical documentation, orders, and patient records.
- Participate in daily care team huddles and collaborate with providers, medical assistants, and clinic staff to deliver evidence-based, patient-centered care.
- Support quality improvement initiatives, professional development activities, and regulatory compliance requirements.
