Job Description:
The Nurse Practitioner (NP) provides high‑quality, patient‑centered care in collaboration with physicians and the clinical care team. This role includes assessing patients, diagnosing conditions, developing treatment plans, prescribing medications, and delivering comprehensive education to support patient health and wellness. The NP ensures care is delivered in accordance with clinic policies, evidence‑based guidelines, and state/federal regulations.
Key Responsibilities:
Perform patient assessments, diagnose, and treat acute and chronic conditions Order and interpret diagnostic tests Develop and manage treatment plans Prescribe medications in accordance with state regulations Educate patients on health maintenance and disease prevention Collaborate with providers and care team to ensure continuity of care Rounding in dialysis centers Seeing patients in clinic
Qualifications:
Active Nurse Practitioner license DEA license and prescriptive authority Nephrology experience preferred
