Location:

) Role summary Provide patient-centered primary and specialty care to residents in skilled nursing and post-acute facilities. Work collaboratively with SNF staff to manage acute and chronic conditions, create treatment plans, and optimize resident outcomes. Schedule and practice model Full-time positions with flexible scheduling Typical facility shift: six hours; rounds usually start between 7:00-10:00 AM No overnight shifts or on-call responsibilities Daily on-site rounds; coordinate care with interdisciplinary teams Clinical responsibilities Evaluate residents, conduct exams, and manage acute and chronic illnesses Prescribe medications and implement treatment plans Order and interpret labs and diagnostics, arrange referrals as needed Communicate with facility staff, families, and external providers Maintain timely, accurate clinical documentation Tools and workflow Streamlined EHR with AI scribe and voice dictation Company-provided iPad with LTE Access to UpToDate® for clinical decision support Compensation and benefits Competitive base pay with uncapped productivity-based upside Up to $50,000 in long-term retention incentives Performance bonuses and potential to exceed industry benchmarks Sign-on bonus and relocation assistance (eligibility applies) Paid Time Off 401(k) with employer match Medical, dental, vision, life, and disability insurance (eligibility applies) CME allowance and licensure reimbursement Company-provided lab coat and device Qualifications Active, unrestricted Nurse Practitioner license required Board certification required (ANCC, AANPCB, PNCB, AACN, or equivalent) Prior experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred To apply Submit CV and license/certification details.
