Job Description

Provide high-quality, patient-centered care to residents in skilled nursing facilities across East Lansing, Michigan, within a collaborative, team-based setting. Seeking Nurse Practitioners and Physician Assistants focused on improving health and quality of life for older adults.
Locations Primary:
East Lansing, MI Additional Michigan:
Carson City, Charlevoix, Crystal Falls, Escanaba, Iron River, Mt. Pleasant, South Haven Nationwide opportunities:
IL, NH, NC, OH, WI
Schedule and clinical responsibilities Full-time positions with flexible scheduling Six-hour facility shifts (rounds typically start between 7:00 a.m. and 10:00 a.m.) No call or overnight responsibilities Conduct daily rounds, evaluate and manage acute and chronic conditions, initiate and adjust medications and treatment plans Collaborate with interdisciplinary teams and facility staff Tools and workflow support Streamlined EHR with AI Scribe and voice dictation Company-provided iPad with LTE Access to UpToDate® for clinical guidance Compensation and benefits Competitive base pay with uncapped, productivity-based upside Up to $50,000 in long-term retention incentives Performance bonuses, sign-on bonus and relocation assistance (eligible candidates) Paid Time Off 401(k) with employer match Medical, dental, vision, life, and disability insurance (eligible providers) CME allowance and licensure reimbursement Company-provided iPad and lab coat Minimum qualifications Active, unrestricted NP or PA license National certification (e.g., ANCC, AANPCB, PNCB, AACN, NCCPA) Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred
