Job Description
Avalon Physician Services is a patient-centered, growth-focused family medicine practice committed to providing exceptional primary care to individuals and families in our community. Our care model emphasizes collaboration, continuity, and compassionate, evidence-based treatment. We are seeking a skilled and dedicated Nurse Practitioner to join our team and work side-by-side with our physician as a clinical partner and physician proxy , delivering high-quality patient care and supporting clinical operations. Position Summary The Nurse Practitioner will provide comprehensive primary care services including evaluation, diagnosis, treatment, education, and follow-up care. In collaboration with the supervising physician, the NP will help manage a shared patient panel, support care coordination, and ensure continuity of care. This role is ideal for a clinician who values autonomy, teamwork, and long-term patient relationships. Key Responsibilities Perform comprehensive assessments including history, physical examination, diagnosis, and treatment planning Manage acute, chronic, and preventive care needs for patients of all ages Prescribe medications, therapies, and durable medical equipment per scope of practice and state regulations Order and interpret laboratory, diagnostic, and imaging tests Collaborate with supervising physician for complex cases and care coordination Provide patient education and counseling on health promotion, disease prevention, and wellness Support clinical protocols, quality improvement initiatives, and care-based metrics Maintain accurate and timely documentation in the EMR Participate in care team meetings and contribute to practice growth and outcomes Serve as a physician extender/proxy to optimize provider efficiency and enhance patient access Qualifications Master's or Doctorate in Nursing with Nurse Practitioner certification (FNP preferred) Current and unrestricted RN and NP licensure in Michigan National Board Certification (AANP or ANCC) Current DEA registration (or ability to obtain)
BLS/ACLS
certification preferred Minimum 1-2 years of experience in primary care or family practice (new graduates with strong clinical experience considered) Excellent communication, clinical judgment, and teamwork skills EMR proficiency and strong organizational abilityPay:
