Job Description
Nurse Practitioner or Physician Assistant Salary:
$90,000 (new grad)- $115,000 (experienced practitioner) per year

Relationship-Based Care:

- What You'll Do
- Conduct in-home clinical assessments and medication reconciliation
- Develop and regularly update Plans of Care in partnership with patients and caregivers
- Diagnose, treat, and manage acute and chronic conditions within scope of practice
- Educate patients and caregivers on conditions, medications, and symptom management
- Collaborate with the Medical Director, interdisciplinary team, and community partners
- Maintain strong clinical documentation and adherence to compliance and continuous quality

Qualifications Required:
- Current licensure as a Nurse Practitioner or Physician Assistant in the State of Michigan
- Practice independently with strong assessment, diagnostic, and care-planning skills
- Excellent communication & relationship-building abilities with patients, families, and teams
- Valid driver's license with ability to travel throughout Berrien, Cass, and Van Buren counties
Preferred:
- Experience providing care in home-based, community, or adult foster care settings
