Perform patient assessments, manage acute and chronic conditions, and develop treatment plans. Order diagnostics, prescribe medications, and educate patients, families, and staff. Collaborate with care teams to ensure seamless, patient-centered care. Maintain accurate documentation and support quality improvement initiatives.
Qualifications:
Current MN NP license, Board Certification, 1+ year related experience preferred, strong geriatric and primary care background, excellent communication skills.
