About Us Aeris Medical Group is a mobile primary care practice delivering high-quality, patient-centered care in Skilled Nursing Facilities (SNFs), Long-Term Care (LTC) centers, and Assisted Living Facilities (ALFs). Our providers are clinicians, educators, and advocates for patients, families, and facility staff. We are expanding across Minnesota and beyond with a mission to transform post-acute primary care. Position Overview We are seeking an experienced Nurse Practitioner to join our growing team. In this role, you will provide comprehensive primary care, manage both acute and chronic conditions, and build meaningful relationships with residents, families, and care teams. This position offers autonomy in clinical practice, a supportive team environment, flexible scheduling, and consistent patient panels. Order and interpret labs/diagnostics; develop and adjust treatment plans. Diagnose, manage, and treat acute and chronic conditions. Prescribe and manage medications per state and federal guidelines. Provide patient, family, and staff education on health and prevention. Collaborate with Aeris team members and facility staff to ensure seamless care. Document accurately and support quality improvement initiatives. Qualifications Current Nurse Practitioner license in Minnesota. Board Certified Nurse Practitioner or Clinical Nurse Specialist 1+ year of experience in a skilled nursing facility or similar setting preferred . Strong background in geriatric, primary, acute, and chronic care. Excellent communication, teamwork, and problem-solving skills. Commitment to patient-centered care, ethics, and continuous learning.
