Physician Assistant (PA-C) or Family Nurse Practitioner (FNP) - Family Medicine (With Urgent Care Add-On)

Pay Rate:
$62.00 - $67.00 per hour (Depending on Experience) About the Role Midwest Urgent Care is seeking a dedicated Physician Assistant (PA-C) or Family Nurse Practitioner (FNP) to join our team in Rogers, MN. This role is primarily focused on Family Medicine and Primary Care , helping establish a dedicated "medical home" for our patients through chronic disease management and routine health maintenance. On occasion, when schedule availability permits, you may also see walk-in urgent care patients. Training is available for laceration repair and IV administration for clinicians looking to refresh or expand these skill sets. This flexible part-time role offers 17 to 26 hours per week across 2 to 3 days, with standard shift hours from 8:00 AM to 5:00 PM.
Key Schedule Details Shift Hours:
8:00 AM - 5:00 PM (Flexible weekday scheduling)
Weekly Hours:
17-26 hours per week (2-3 days per week)
Weekend Requirement:
Required to work 1 Saturday per month Key Responsibilities Primary Care & Health Maintenance (Main Focus): Perform annual wellness exams, preventive health screenings, and routine primary care for patients of all ages. Chronic Care Management (CCM): Act as the primary clinician for patients with complex, long-term conditions (e.g., Diabetes, Hypertension, COPD, Asthma, CKD). Secondary Urgent Care (As Time Permits): Evaluate and treat acute walk-in urgent care conditions when there is availability between primary care appointments.
Clinical Procedures & Diagnostics:
Perform or learn routine outpatient procedures (laceration repair/suturing, basic wound care, and IV administration—training provided) and order/interpret diagnostic tests, labs, and imaging.
Patient-Centered Care:
Address holistic health needs with an exceptional bedside manner, compassionate communication, and in-depth patient education.
Documentation & Coordination:
Maintain accurate EHR documentation using standard
CPT/ICD-10
coding and coordinate care with specialists as needed.
Qualifications & Requirements Education:
Graduate of an accredited Physician Assistant (PA) or Nurse Practitioner (NP) program.
Licensure:
Active, unrestricted license to practice in Minnesota and active DEA registration in good standing.
Experience:
Minimum of 1 year of clinical experience in Family Medicine or Internal Medicine.
Soft Skills:
Outstanding communication skills, an empathetic bedside manner, and a willingness to learn minor clinical procedures (laceration repair and IV administration).
