What You'll Be Doing Provide comprehensive primary care services throughout assisted living, memory care, independent living, and skilled nursing communities. Diagnose, treat, and manage acute illnesses and chronic medical conditions. Perform comprehensive assessments, annual wellness visits, and routine follow-up care. Develop individualized treatment plans focused on improving long-term patient outcomes. Coordinate care following hospitalizations and specialty consultations to reduce avoidable readmissions. Collaborate daily with physicians, nurses, therapists, pharmacists, social workers, and senior living leadership. Educate patients, families, and caregivers while serving as a trusted clinical resource. Identify opportunities to improve quality measures, patient satisfaction, and continuity of care.
