Job Description
Nurse Practitioner - Home Visits & Assisted Living Facilities Golden Years Geriatrics is seeking a compassionate and motivated Nurse Practitioner to provide primary care services to patients in Assisted Living Facilities, independent living communities, and private homes throughout Mississippi. This is a field-based position with primary coverage in the Southaven and Tupelo areas, with opportunities to serve patients throughout the state. Providers enjoy a flexible schedule and the ability to make a meaningful impact in the lives of seniors by delivering healthcare directly where they live.
Responsibilities:
Perform comprehensive patient assessments and physical examinations Diagnose and treat acute and chronic medical conditions Provide preventive care and chronic disease management Prescribe medications and order diagnostic testing as appropriate Develop and manage individualized treatment plans Coordinate care with facility staff, caregivers, specialists, and other healthcare professionals Complete timely and accurate medical documentation Participate in care coordination and follow-up servicesQualifications:
Active Mississippi Nurse Practitioner license National Board Certification DEA license or ability to obtain Strong clinical, communication, and organizational skills Experience in geriatrics, primary care, family medicine, internal medicine, or home health preferred Reliable transportation and willingness to travelCompensation:

Providers will serve patients in Assisted Living Facilities and private homes throughout Mississippi, with primary coverage in: Southaven and surrounding communities Tupelo and surrounding communities Additional travel throughout Mississippi may be required based on patient needs.Job Type:
