Job Description

As an Advanced Practice Provider at CVS Health in the Healthcare & Medical Services department, you will deliver high-quality primary and urgent care in a patient-centered, team-based environment. You'll assess, diagnose, and treat a broad range of acute and chronic conditions, prescribe medications, and educate patients on prevention and wellness. Collaborating closely with pharmacists, physicians, and care teams across CVS Health, you'll leverage integrated clinical tools and EHR systems to coordinate care and improve outcomes. Responsibilities Deliver comprehensive primary and urgent care, including assessment, diagnosis, and treatment of acute and chronic conditions Perform focused examinations, order and interpret diagnostic tests, and develop evidence-based care plans Prescribe medications and treatments in alignment with state scope-of-practice and CVS Health clinical protocols Provide patient education, preventive care, and health coaching to improve outcomes and adherence Collaborate with physicians, pharmacists, nurses, and care teams across CVS Health services and community partners Document all encounters accurately and timely in the Electronic Health Record (EHR) to support quality and compliance Participate in quality improvement, population health, and value-based care initiatives Maintain licensure, certifications, and continuing education, contributing to a culture of continuous learning and inclusion Required Skills Patient assessment and diagnosis Primary and urgent care management Electronic Health Record (EHR) documentation Prescriptive authority and medication management Chronic disease management (e.g., diabetes, hypertension) Minor procedures (e.g., suturing, wound care) Clinical decision-making and differential diagnosis Care coordination and referral management Patient education and health coaching Regulatory and compliance documentation
