Job Description

HouseCalls Nurse Practitioner - The Judge Group is Hiring The Judge Group is seeking experienced Nurse Practitioners to perform in‑home health assessments for Medicare Advantage members. This role offers flexible scheduling, weekend availability, and meaningful one‑on‑one patient care. Requirements Minimum 6 months NP experience (no new grads).
National Board Certification:
AANP or ANCC.
Accepted Specialties:
Adult Gerontology Primary Care, Adult NP, Family NP, Gerontology NP, Adult Gerontology Acute Care NP (state‑dependent).
Not Accepted:
Mental Health, Pediatric, Women's Health/OBGYN, Psych, Telephonic‑only NP backgrounds.
Autonomous License Required in:
CT, FL, IL, MN, NE, NY, SD, VA, VT.
Acute Care NP approval only in:
AK, AZ, CO, DE, DC, HI, ID, IA, KS, KY, ME, MD, MA, MI, MO, MT, NV, NH, NJ, NM, ND, OK, OR, RI, UT, VT, WA, WV, WY.
Role Overview Nurse Practitioners conduct 45-90 minute in‑home assessments including: Medical history, ROS, physical exam, vitals Medication review & depression screening Identification of diagnoses and care gaps Patient education and referral identification Expectations Chart completion within 48 hours. Documentation discrepancies & addendums due within 7 calendar days. Maintain 6 weeks of availability in practitioner portal. Pre‑requisites must be completed before training; class dates cannot be changed. Must pass background, drug screen, TB test. Paid per assessment; no mileage reimbursement. Must call members the day before visits (unpaid). Assessments may be returned for corrections.

Rural, urban, and complex housing environments Pets, smoking, hoarding, pests, mold Weather‑related travel, variable road conditions, parking challenges Reliable transportation required. Safety Support High‑crime area review team Optional Noonlight app Geo‑fencing & "Do Not Visit" designations Required Documents for Approval Resume Attestation form Active NP license in good standing
