Description Advanced Practice Provider (NP/PA) - Transitions Care | UNC Wayne -
Goldsboro, NC Job Summary:
UNC Health is seeking an experienced Advanced Practice Provider (APP) to support care transitions in the Wayne region. This role will focus on post-discharge patient care, ensuring timely follow-up and continuity across care settings to improve outcomes and reduce readmissions. In addition to managing transitions-of-care visits, the APP will provide coverage for overflow acute care appointments as needed. This position will be based in the Goldsboro area.Responsibilities:
Provide comprehensive post-discharge follow-up care for patients transitioning from hospital to outpatient settings Perform patient assessments, including history, physical examination, and medication reconciliation Develop and implement individualized care plans in collaboration with physicians and care teams Coordinate care with primary care providers, specialists, and community resources Educate patients and families on treatment plans, medications, and self-care strategies Manage overflow same-day or acute care visits as needed Support initiatives to improve care transitions, patient outcomes, and reduce hospital read missionsQualifications:
Graduate of an accredited Nurse Practitioner or Physician Assistant program Current North Carolina licensure (or eligible) as an NP or PA Minimum of 2 years of clinical experience requiredPreferred Qualifications:
