This innovative model combines team-based care, modern technology, convenient access, and integrated in-person and virtual services—including urgent care—to create a more connected and efficient approach to primary care. Responsibilities Provide comprehensive outpatient primary care to adult patients Diagnose and manage acute and chronic medical conditions Develop evidence-based treatment plans and provide preventive care Collaborate with an interdisciplinary care team to coordinate patient services Utilize virtual care and modern technology to improve access and continuity Educate patients on preventive health, chronic disease management, and wellness

Compensation & Benefits Salary:
