Rite Med Group is collaborating is seeking a full time , experienced, Nurse Practitioner or Physician Assistant for a post-acute rehabilitation, long-term care, and memory care center near Egg Harbor Township, NJ We offer great benefits such as PTO, sick hours, paid holidays, health insurance, paid 401 (k) retirement benefits! Flexible schedule, no weekends or on call required! The pay range for the Nurse Practitioner position is: $150,000.00 - $180,000.00 per year.

Responsibilities:

Provide direct patient care, assessments, and treatment for residents and short-term rehab patients Collaborate with attending physicians, nursing staff, and therapy teams on care planning Manage medically complex patients within our specialty clinical programs Support hospital readmission prevention and smooth care transitions Document thoroughly and communicate with families as part of a resident-centered approach Discussing and reviewing patients' medical history, symptoms, allergies, and current medications

Requirements:

EMR experience Licensed NP/PA in NJ At least 1 year experience in an urgent care setting or skilled nursing home experience (Required)
