As a member of the interdisciplinary team, theNurse Practitioner will provide meaningful, end-of-life care where it matters mostat home, in nursing facilities, and at our Kaplan Family Hospice Residence. M-F, 8:30am to 4:30pm. One (1) weekend on call per month. One (1) to two (2) holidays on-call per year.Responsibilities may include:
Conducting in-home patient visits for symptom management, collaboration of care, goals of care discussion, assessment of complex wounds, and evaluation of eligibility for hospice benefits.In collaboration with the medical director and intake nursing staff, reviewing clinical documents to determine eligibility and terminal diagnosis.
