$115,000-$128,000 based upon experience About the Opportunity Our client, a respected regional healthcare organization, is seeking two Family Nurse Practitioners to join its outpatient primary care team. This position offers the opportunity to build long-term patient relationships while practicing in a collaborative, community-focused environment supported by a larger integrated healthcare network. Key Responsibilities Provide comprehensive primary care services across all stages of life in an outpatient setting. Diagnose, treat, and manage acute and chronic health conditions. Perform preventive care, wellness visits, patient education, and chronic disease management. Build lasting relationships with patients and families through continuity of care. Collaborate with physicians, advanced practice providers, nursing staff, and interdisciplinary teams. Maintain accurate clinical documentation using the electronic medical record (EMR). Participate in quality improvement initiatives and promote evidence-based care. Minimum Qualifications Licensed Family Nurse Practitioner (FNP). Active RN license or eligibility for RN licensure in the State of New York. At least one year of experience is preferred in primary care, family medicine, internal medicine, or community health.
