Job Description

Universal Primary Care is hiring a Nurse Practitioner in Salamanca, NY. This is a full-time position. Key Responsibilities Provide comprehensive primary care, including assessment, diagnosis, treatment, and follow-up.

Manage acute and chronic conditions, including diabetes, hypertension, cardiovascular disease, respiratory conditions, and other common primary care needs.

Perform preventive health visits, screenings, immunizations, and patient education.

Order and interpret laboratory, imaging, and other diagnostic studies.

Prescribe medications and therapeutic interventions within scope of practice.

Coordinate care with specialists, behavioral health providers, hospitals, and community resources.

Maintain accurate and timely documentation in the electronic health record.

Participate in quality improvement, population health, and clinical performance initiatives.

Collaborate with the interdisciplinary care team to address medical and social factors affecting patient health.

Support the health center's commitment to accessible, equitable, culturally responsive care.
