A Nurse Practitioner provides primary care and nursing care to patients. Performs physical exams, orders diagnostic tests, and provides medications. Works in a physician's office or other healthcare facility, under the direction of a physician. Performs work similar to that of a physician assistant but has different medical training and degree.
Position Overview We are seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join the Senior Community Care (SCC) program for a contract assignment of approximately 3 months. In this role, you will deliver longitudinal, integrated clinical care to high-risk health plan members across two facility settings in Oneida County, NY. This position offers strong work-life balance with flexible scheduling, no on-call responsibilities, no weekends, and no holidays required.

Medical attire Primary Responsibilities Conduct comprehensive health assessments for high-risk members within facility settings. Manage acute and chronic medical and behavioral health conditions as part of an interdisciplinary care team. Ensure accurate, timely, and complete clinical documentation, including ICD-10 diagnostic conditions. Facilitate advanced illness and end-of-life planning conversations with members and families. Provide patient and caregiver counseling, education, and clinical support. Communicate and collaborate continuously with interdisciplinary care team members.
Required Qualifications Education & Licensure:
NP:
Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program. Active, unrestricted NP license in New York (or ability to obtain prior to start).
PA:
Graduate of an accredited Physician Assistant degree program. Active, unrestricted PA license in New York (or ability to obtain prior to start).
Board Certification:
NP:
Board certified via AANP or ANCC (FNP, ANP, GNP, or AGACNP).
PA:
Board certified via NCCPA.
Licensure & Clearance:
Active DEA licensure / prescriptive authority, ability to obtain a Collaborative Practice Agreement (if applicable), and ability to pass a pre-employment drug screening.
Physical & Logistics Requirements:
Access to reliable transportation for travel between facilities during shifts; ability to perform physical assessments, navigate stairs, lift up to 30 lbs, and work across varied dwelling conditions.

vaccination is required prior to employment. Preferred Qualifications 1+ years of clinical practice experience (long-term care setting preferred). Experience managing complex behavioral, social, and functional health needs in geriatric populations. Strong background in chronic illness management, geriatrics, and advanced care planning.
