THE ROLE

Support seniors in assisted living communities. Low-acuity patient population Conduct psychiatric evaluations and follow-up visits Provide medication management Collaborate with facility staff and primary care providers Maintain timely and accurate clinical documentation

REQUIREMENTS

Help seniors age with grace, dignity, and purpose. Active NY License Active DEA Medicare Enrollment 2+ Years PMHNP Experience Strong clinical, communication, and documentation skills
