Nurse Practitioner (NP) / Physician Assistant (PA) - Full-Time Join a growing healthcare team dedicated to compassionate, patient-centered care. We are actively hiring experienced Nurse Practitioners (NPs) / Physician Assistants (PAs) to provide comprehensive care across private Skilled Nursing Facilities (SNF)throughout the Utica area. If you are a motivated, detail-oriented clinician who thrives in a fast-paced environment and values teamwork, this is an excellent opportunity to grow with a practice that truly supports its providers. Responsibilities Conduct thorough patient assessments and develop individualized care plans Diagnose, manage, and treat both acute and chronic conditions Monitor patient progress and adjust treatment plans as needed Document all visits using Electronic Health Record (EHR) systems Collaborate with physicians, nursing staff, and interdisciplinary teams Educate patients and families on health management and wellness strategies Responsible for active communication with the facility Work from Monday through Thursday from 9 am to 4 pm Maintain compliance with all healthcare regulations and HIPAA standards Skills & Competencies Strong diagnostic and clinical judgment skills Proficient in medication management and post-acute care protocols Familiarity with infection prevention and chronic disease management Excellent communication, teamwork, and patient education skills Highly organized and efficient in managing multiple patients daily Comfortable working independently within facility settings Qualifications Licensed and Board-Certified NP or PA in New York State Authorized to work in the United States Previous experience in SNF ( Required ) Strong work ethic and commitment to high-quality care Proficiency with EHR systems (e.g., Sigmacare, PointClickCare, or similar) Fluent in English (additional languages a plus) What We Offer Competitive pay commensurate with experience Comprehensive support team to streamline documentation and communication Consistent weekday schedules with flexibility for coverage preferences Opportunity for career advancement Collaborative environment with mentorship from experienced providers Ready to take the next step in your healthcare career?
