Job Description

Join our expanding clinical team delivering patient-centered care across skilled nursing facilities in Lyndhurst, Ohio. We seek compassionate Nurse Practitioners and Physician Assistants committed to improving the health and quality of life of older adults within a collaborative, interdisciplinary environment. New graduates are encouraged to apply. Key responsibilities and schedule Full-time positions with flexible scheduling and six-hour facility shifts (rounds typically begin between 7:00 a.m. and 10:00 a.m.) No on-call or overnight obligations Perform daily rounds, manage acute and chronic conditions, initiate and adjust medications and treatment plans, and coordinate care with facility staff and families Efficient documentation using a streamlined EHR with AI Scribe, voice dictation, and a company-provided iPad with LTE Access to UpToDate® for point-of-care clinical guidance Compensation and benefits Highly competitive, uncapped productivity-based compensation with potential to exceed industry benchmarks Up to $50,000 in long-term retention incentives and additional performance bonuses Sign-on bonus and relocation assistance for eligible candidates Paid time off; 401(k) with employer match Medical, dental, vision, life, and disability insurance for eligible providers CME allowance, licensure reimbursement, company-provided iPad with LTE, and lab coat Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA) Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred Join us to make a meaningful impact on older adult care in a supportive, team-focused practice.
