Job Description
You are applying for a position through Medix, a staffing agency. The actual posting represents a position at one of our clients. Job Summary Our client is seeking a Primary Care Nurse Practitioner to provide primary and preventative care for long-term care patients at skilled nursing facilities. The role involves managing residents with acute changes in condition, conducting advanced care planning, and chronic care management alongside other healthcare professionals. Key Responsibilities Provide primary and preventative care for patients in long-term care. Manage residents experiencing acute changes in their condition. Conduct advanced care planning and manage chronic care. Prescribe appropriate medications, including controlled substances in accordance with Ohio Law. Order, review, and follow-up on treatments and diagnostic testing. Communicate regularly with nursing home staff regarding patient care and changes in condition. Collaborate with primary care physicians. Qualifications Current, unrestricted NP license in the state of Ohio. Board Certification. DEA licensure. Clinical background in adult, family, or geriatric specialties. Additional Requirements Experience working in a long-term care setting. Ability to work with an EMR. Experience managing patients with advanced illness and multiple chronic conditions. Ability to work within an interdisciplinary care team to coordinate patient care. Psych experience is a plus.
