POSITION OVERVIEW
The Family Nurse Practitioner (FNP) provides comprehensive, patient-centered primary care to individuals and families across the lifespan in a community health center setting. The FNP is responsible for assessing, diagnosing, treating, and managing acute and chronic health conditions while emphasizing preventive care, health education, care coordination, and improved health outcomes. The ideal candidate is committed to serving diverse and underserved populations and embraces a team-based model of care. The FNP works collaboratively with physicians, nurses, behavioral health professionals, care coordinators, pharmacists, and other members of the health care team to provide accessible, high-quality, culturally responsive care.
LOCATION
This position will work at our School-Based Health Center located at 700 Lemoyne Rd, Northwood, OH.

ESSENTIAL DUTIES
Essential Duties and Responsibilities:
Provide comprehensive primary care services to pediatric, adolescent, adult, prenatal, and/or geriatric patients, consistent with clinical training, experience, and scope of practice. Obtain comprehensive medical histories and perform physical examinations. Diagnose and manage acute illnesses, chronic diseases, and common primary care conditions. Develop, implement, and evaluate individualized treatment plans in collaboration with patients and other members of the care team. Prescribe medications and therapeutic interventions in accordance with applicable laws, regulations, organizational policies, and clinical guidelines. Order, interpret, and follow up on appropriate laboratory tests, diagnostic studies, and other medical procedures. Provide preventive health services, including health screenings, immunizations, wellness examinations, and patient education. Manage and monitor chronic conditions such as diabetes, hypertension, hyperlipidemia, asthma, and other common primary care diagnoses. Provide counseling regarding nutrition, exercise, tobacco cessation, preventive health, medication adherence, and other health-related behaviors. Recognize and appropriately manage urgent and emergent medical conditions, including arranging appropriate referral or transfer when indicated. Make referrals to specialists and other health care providers and coordinate care as appropriate. Participate in a multidisciplinary, team-based approach to patient care. Establish and maintain effective therapeutic relationships with patients and families. Provide culturally sensitive and trauma-informed care to patients from diverse backgrounds and populations. Document patient encounters accurately, thoroughly, and timely in the electronic health record. Complete required medical records, orders, referrals, prescriptions, and other clinical documentation in accordance with organizational standards. Participate in quality improvement, population health, and patient safety initiatives. Support community health center performance goals and applicable quality measures. Participate in clinical meetings, peer review, and other organizational activities as required. Maintain current knowledge of evidence-based primary care practices, clinical guidelines, and applicable federal and state regulations. Participate in after-hours phone coverage rotation, as assigned. Participate in continuing education and professional development activities. Comply with all organizational policies and procedures, including standards related to patient privacy, infection prevention, safety, and regulatory compliance. Perform other duties as assigned within the scope of the position.
REQUIRED QUALIFICATIONS
Required Education Master's degree in Nursing from an accredited nurse practitioner program. Graduate-level preparation as a Family Nurse Practitioner (FNP) or equivalent nationally recognized family nurse practitioner certification. Required Minimum Qualifications At least one (1) year prior experience providing primary care as a Family Nurse Practitioner. Preferred Experience Experience working with medically underserved, rural, urban, or other diverse patient populations is preferred. Experience in a community health center, federally qualified health center, or similar setting is preferred. Required Licensure and Certification Current, unrestricted Registered Nurse (RN) license issued by the Ohio Board of Nursing. Current, valid Ohio Advanced Practice Registered Nurse (APRN) license/ designation as a Certified Nurse Practitioner (CNP) in accordance with Ohio Revised Code Chapter 4723 and applicable Ohio Administrative Code rules. Current national certification as a Family Nurse Practitioner (FNP) through a nationally recognized certifying organization, such as the American Nurses Credentialing Center (ANCC) or American Association of Nurse Practitioners Certification Board (AANPCB). Current Drug Enforcement Administration (DEA) registration, or eligibility to obtain and maintain DEA registration, as required by the position and scope of prescribing practice. Current Basic Life Support (BLS) certification. Must meet all applicable Ohio requirements for prescribing and practicing within the nurse practitioner's authorized scope of practice. Must maintain ability to be credentialed with all required health insurance plans. Valid Ohio Driver's License. License must be maintained throughout employment. Required Ongoing Compliance Employment is contingent upon maintaining all licenses, certifications, registrations, and authorizations required for the position throughout employment. The FNP must immediately notify the organization of any restriction, lapse, suspension, investigation, or other change affecting the individual's authority to practice or prescribe in Ohio.
The FNP must comply with:
Ohio Revised Code Chapter 4723. Written Standard Care Arrangement (SCA) with collaborating physician(s), as applicable, that satisfies the requirements of Ohio Revised Code Section 4723.431 and Ohio Administrative Code Chapter 4723-8. Applicable rules of the Ohio Administrative Code, including Chapter 4723-8 governing APRN certification and practice and Chapter 4723-9 governing prescribing by APRNs. Requirements of the Ohio Board of Nursing. Applicable federal and state controlled-substance requirements. Applicable Medicaid, Medicare and other payer requirements. Community health center policies and clinical protocols. All applicable federal, state, and local laws governing the delivery of health care.

Complies with mandated reporting laws. Complies with requirements of the Health Insurance Portability and Accountability Act. Logs and maintains necessary training records. Regular and punctual attendance. Strong clinical assessment, diagnostic, treatment-planning, and decision-making skills. Knowledge of evidence-based primary care and preventive health practices. Ability to independently manage a broad range of primary care conditions while recognizing when physician consultation or specialty referral is appropriate. Excellent communication and interpersonal skills. Ability to establish trust and rapport with patients and families. Commitment to culturally responsive, equitable, and patient-centered care. Ability to work effectively as part of a multidisciplinary health care team. Strong organizational skills and attention to detail. Proficiency with electronic health records and standard clinical technology. Commitment to quality improvement, patient safety, and continuous professional development.
