We are seeking a skilled Nurse Practitioner to support home health risk assessment visits for Medicare Advantage members living throughout Belmont County, OH. In this per diem role, you will have the flexibility to design your own schedule, with opportunities to complete annual health assessments and post-discharge visits in communities such as Richland Township, Pease Township, and Pultney Township. Each comprehensive assessment typically requires about 45 minutes, allowing you to deliver essential care and important education to members in their homes.

Key Responsibilities:

Complete thorough home health risk assessments, focusing on medical history, conditions, and medication Educate members about their health conditions and medication regimens Identify and address gaps in care, recommending actionable steps Visits do not include prescribing or creation of treatment plans

Qualifications:

Active Nurse Practitioner license in Ohio Current board certification through AANP or ANCC Current BLS (CPR) certification Master of Science in Nursing (MSN) from an accredited institution At least 6 months of recent NP experience preferred This position provides a compensation range of $80-$90 per completed evaluation, with additional mileage reimbursement and compensation for administrative time.
