We're looking for a Family Nurse Practitioner to join our collaborative team. You'll focus on early intervention, chronic disease management, and long-term wellness in a setting that values quality over volume. What You'll Do (Key Responsibilities in Patient Care):Comprehensive Care:
Provide both in-person and virtual visits to increase access and continuity of care.Diagnosis & Planning:
Prescribe medications and develop personalized health plans with your patients.Wellness Education:
Educate patients on wellness, prevention, and lifestyle management.Collaboration:
Collaborate closely with physicians, medical assistants, and other clinicians.Culture:
Contribute to a positive, team-oriented clinic culture. What You'll Bring to the Team (Minimum Qualifications): Active and unrestricted FNP license in the practicing state. Board certification as anFNP. DEA
registration. 2+ years of primary care experience preferred. Experience treating patients ages 2+. Familiarity with HIPAA and a strong commitment to patient confidentiality.
