Job Description
Nurse Practitioner / Physician Assistant Internal Medicine and Geriatrics Ponca City, Oklahoma Moscati Medical Group, in partnership with Sage Integrated Health, is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing Internal Medicine and Geriatrics team in Ponca City, Oklahoma. This is not just another primary care job. It's an opportunity to help redefine how healthcare is delivered to older adults. Our practice is uniquely embedded within a Continuing Care Retirement Community (CCRC), allowing you to care for patients across the entire continuum of aging.
Your practice will include:
- Outpatient internal medicine and geriatrics clinic
- Skilled Nursing Facility (SNF)
- Long-Term Care (LTC)
- Assisted Living
- Memory Care
