Nurse Practitioner / Physician Assistant Internal Medicine and Geriatrics Ponca City, Oklahoma Moscati Medical Group, in partnership with Sage Integrated Health, is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing Internal Medicine and Geriatrics team in Ponca City, Oklahoma. Our practice is uniquely embedded within a Continuing Care Retirement Community (CCRC), allowing you to care for patients across the entire continuum of aging.
Your practice will include:
  • Outpatient internal medicine and geriatrics clinic
  • Skilled Nursing Facility (SNF)
  • Long-Term Care (LTC)
  • Assisted Living
  • Memory Care
