We are creative and engaging.
Our work environment includes:
Modern office setting Work-from-home days We are a Mobile Clinic that visits patient's home, board and care, assisted living facilities and skilled nursing. Candidate will do the following : You will be assigned to a City of your choice or near your Home/ Work covering multiple locations and working with different teams to manage patients at their homes You will deliver proactive preventive care at patients' homes to reduce unnecessary hospitalizations visits. Partner with the patient's primary care provider and specialists to deliver integrated care Diagnose and treat patients, consistent with standard of care You will be conducting comprehensive exams and creating care plans for the most vulnerable, senior elderly Medicare patients Providing appropriate education and counseling to patients and their families/caregivers Documenting appropriately in EHR and other systems Collaborating with other members of the care team to manage the needs of the patient This is a fun and rewarding type of Job.
