125k - 185k (Calculated by visit, see description for more info) Hybrid | Full Time/Part Time | Astoria About Rockwell Health Center is growing our endocrinology program and hiring a Nurse Practitioner (NP) to lead Type 2 Diabetes and metabolic care for our Brooklyn patients. Hybrid outpatient and telehealth, focused on long-term diabetes management, glycemic control, and complication prevention. What you'll do Manage Type 2 Diabetes and metabolic conditions: assessment, treatment planning, ongoing care Initiate and adjust pharmacologic therapy (insulin, GLP-1 receptor agonists, oral agents) Order and interpret labs (A1C, lipids, renal function) and adjust treatment Identify and proactively manage diabetes-related complications, including neuropathy, retinopathy, nephropathy, and cardiovascular risk Deliver patient education on lifestyle modification: nutrition, physical activity, weight management, and medication adherence Coordinate care with the endocrinology team, primary care, dietitians, and the broader care team Use telehealth and CGM (continuous glucose monitor) data for monitoring and follow-up Support quality measures including A1C control and HEDIS (Healthcare Effectiveness Data and Information Set) metrics Requirements Master's or Doctoral NP degree from an accredited program Active, unrestricted New York State NP license Board certification (FNP, AGNP, or ANP) DEA registration and active malpractice insurance Active enrollment with Medicare (PECOS) and New York Medicaid (eMedNY) as a billing provider 2+ years in endocrinology, primary care, or chronic disease management preferred Preferred skills CGM data interpretation and insulin pump therapy Bilingual Spanish HEDIS or value-based care familiarity How compensation works Pay is per completed visit/note: $35.00 per visit for the first 20 visits per day, $40.00 per visit after that. The annual range above reflects expected earnings at typical visit volumes. First two weeks are paid training at $75.00/hour or per-visit, whichever is higher.
