Job Description
The Nurse Practitioner provides comprehensive outpatient primary care services to patients of ages while delivering compassionate, evidence-based care in a collaborative clinical environment. The NP is responsible for evaluating, diagnosing, treating, and educating patients, promoting preventive healthcare, managing chronic conditions, and coordinating care to patient outcomes. This role works closely with physicians, medical assistants, and administrative staff to ensure high quality, patient-centered care. Essential Duties & Responsibilities Provide comprehensive primary care services to patients 2 years of age through 65+ years. Evaluate, diagnose, and treat acute and chronic medical conditions. Perform comprehensive physical examinations, annual wellness visits, preventive health screenings, and follow-up appointments. Conduct well-child examinations beginning at 2 years of age. Develop individualized treatment plans based on evidence-based clinical guidelines. Order, interpret, and review laboratory tests, diagnostic imaging, and other diagnostic studies. Prescribe medications and treatments in accordance with California Law. Manage chronic diseases through ongoing assessment, treatment, and follow-up care. Educate patients and families regarding diagnoses, medications, treatment plans, preventive care, nutrition, and healthy lifestyle choices. Review laboratory, diagnostic results, communicate findings and recommendations to patients in a timely manner. Document all patient encounters accurately using Tebra and Office Ally electronic health record (EHR) systems. Complete and finalize all patient documentation and clinical notes within 24 hours of each patient encounter. Process electronic prescriptions, referrals, prior authorizations, and patient communications through the EHR. Maintain compliance with HIPAA and all applicable federal, state, and organizational regulations. Participate in provider meetings and quality improvement initiatives. Perform other duties as assigned to support clinic operations and patient care. License & Certifications Active California Nurse Practitioner (NP) License Board Certification through the American Academy of Nurse Practitioners (AANP) Current Basic Life Support (BLS) Certification. Valid Drug Endorcement Administration (DEA) Registration. Active National Provider Identifier (NPI) Qualifications One (1) or more years of experience in Family Practice, Internal Medicine, Primary Care, or Urgent Care preferred. Experience using Tebra and Office Ally electronic health record systems preferred. Fluent in English (required) Strong clinical judgment, communication, and organizational skills Ability to work independently while collaborating effectively with physicians and clinical staff. Work Schedule Full-Time Monday through Friday Day Shift Schedule may vary based on clinic needs and business demands. Weekend availability may be required in the future as clinic operations expand. Work Location This is an in-person position. Remote work is not available.
