Job Description
Senior Care Therapy seeks a Psychiatry Nurse Practitioner to provide compassionate mental health care to seniors in long-term care, assisted living, and community settings. In this hybrid role, you will conduct psychiatric evaluations, manage medications, and develop individualized treatment plans using both on-site and telehealth visits. You will collaborate with an interdisciplinary team and partnering facilities to improve residents' emotional well-being, safety, and quality of life. Responsibilities Perform comprehensive psychiatric evaluations for seniors in long-term care, assisted living, and community settings. Develop and manage individualized treatment and medication plans for geriatric patients. Provide follow-up psychiatric care via on-site visits and telehealth platforms. Collaborate with interdisciplinary teams and facility staff to coordinate holistic patient care. Monitor treatment response, adjust medications, and document care in the EHR in a timely manner. Educate patients, families, and facility staff on mental health conditions and treatment options. Identify and manage psychiatric crises, including risk assessments and safety planning. Ensure compliance with regulatory standards, facility policies, and documentation requirements.
