Responsibilities Perform comprehensive psychiatric evaluations for seniors in long-term care, assisted living, and community settings. Develop and manage individualized treatment and medication plans for geriatric patients. Provide follow-up psychiatric care via on-site visits and telehealth platforms. Collaborate with interdisciplinary teams and facility staff to coordinate holistic patient care. Monitor treatment response, adjust medications, and document care in the EHR in a timely manner. Educate patients, families, and facility staff on mental health conditions and treatment options. Identify and manage psychiatric crises, including risk assessments and safety planning. Ensure compliance with regulatory standards, facility policies, and documentation requirements.
