Job Description
Nurse Practitioner (Telehealth) - Bilingual Spanish Required We are hiring a Nurse Practitioner to conduct Medicare Annual Wellness Visits and evaluation and management visits by telehealth for patients across Illinois. Our patients are older adults on Medicare, many of whom are managing multiple chronic conditions and need help navigating their care. A meaningful share of them speak only Spanish, so fluency in both languages is required for this role. Schedule & Structure 1099 Contract 100% remote telehealth [Hours, e.g. Mon-Fri 9am-5pm CT] with a set weekly schedule Visits are scheduled and prepped by our team, so you spend your time with patients rather than chasing them Compensation $45 - $60 per hour What You'll Do Perform Medicare Annual Wellness Visits including health risk assessments, cognitive screening, medication review, and a personalized prevention plan Conduct E/M visits for new and established patients Identify patients who would benefit from our navigation program and set the plan of care our team then executes Document in [EHR] to Medicare standards, with clean, timely notes Coordinate with our patient advocates on follow-up needs, referrals, and equipment Communicate directly with Spanish-speaking patients and their families without an interpreter Requirements Active Illinois APRN license in good standing, FNP or AGNP preferred National certification (AANP or ANCC) Fluent in Spanish and English, spoken and written, at a level that supports clinical conversation with patients and families Enrolled in Medicare as an individual provider (PECOS/855I) or willing to enroll and reassign benefits to our group Experience with Medicare Annual Wellness Visits strongly preferred Comfortable working independently by phone and video with an older adult population Reliable high-speed internet and a private, HIPAA-appropriate workspace Preferred Geriatrics, primary care, or home-based primary care background Prior telehealth experience Familiarity with Medicare preventive services and chronic care management or care navigation programs Additional state licenses or Nurse Licensure Compact eligibility, as we expand beyond Illinois We review applications within 48 hours.
