Primary Care Nurse Practitioner Primarily In Person Optional Telehealth Choose Your Schedule 1099 Independent Contractor Resolution Health | Portland/Gresham | Nonexclusive Engagement Build a Practice That Reflects Your Ambition You have invested in becoming a capable clinician. Your next opportunity should offer more than a fixed schedule, a fixed hourly ceiling, and limited influence over how you practice. Resolution Health is seeking a Family Nurse Practitioner or Adult-Gerontology Primary Care Nurse Practitioner to provide primarily in-person adult primary care, with the option to incorporate telehealth. This opportunity combines generous production-based compensation, clinician-selected scheduling, company-arranged clinical space, and the potential to help develop an expanding clinician-led practice. We are looking for individuals who are resolute in their standards, bold in improving care, dedicated to their patients, and enterprising enough to help build something meaningful . Your Schedule, Your Appointment Lengths Resolution Health provides access to shared clinical space during defined availability windows. You choose whether and when to work within those windows. The available hours are not assigned shifts. Choose your working days, start and end times, and appointment lengths. Use 15-, 20-, or 30-minute appointments, longer visits, or a combination that fits your clinical judgment and workflow. There are no minimum weekly hours, mandatory evening or weekend shifts, or exclusivity requirements . You may maintain another professional role and begin with limited availability. You are not required to remain onsite when no accepted appointment or clinical responsibility requires your presence. Optional telehealth sessions may complement your in-person schedule, subject to approved work locations, patient-state licensure, credentialing, and coverage. Accepted appointments and outstanding patient-care responsibilities still require timely completion or appropriate transfer. Schedule autonomy comes with professional accountability. Generous, Transparent Production-Based Compensation Compensation is paid through a written per-service fee schedule rather than a fixed hourly wage or percentage of collections. No-show compensation is earned only after the patient charge is actually collected and retained, subject to the agreement's refund, reversal, and recoupment provisions. Illustrative Earning Potential A favorable, fully utilized schedule can generate gross clinical-service fees approximately as follows: Clinician-selected appointment length Completed appointments per clinical hour Illustrative gross fees per fully utilized clinical hour 20-minute appointments 3 Approximately $180 15-minute appointments 4 Approximately $220 30-minute appointments 2 Approximately $150 These examples reflect different modeled visit mixes, including supported Level 3 and Level 4 encounters, eligible longitudinal-care and after-hours services, and separately supportable preventive or wellness services during a portion of longer appointments. They do not assume that every visit qualifies for every add-on. These are not guaranteed hourly rates or promises of patient volume. They describe fees during fully utilized clinical scheduling blocks, not earnings per hour of all work performed. Actual earnings depend on completed volume, patient and payer mix, supported coding, appointment length, cancellations, scheduling gaps, documentation, and follow-up responsibilities. Taxes and contractor expenses also affect net income. There is no requirement to use shorter appointments or meet higher-code quotas. Appropriate consultation, referral, escalation, and lower-level coding are fully supported. Practice Support Without Taking on the Base Office Rent Resolution Health provides: Company-arranged shared clinical space , with the base rental expense paid by the practice. Professional liability coverage for approved services , subject to the applicable policy and disclosed coverage terms. Credentialing, scheduling, billing, and technology support , including approved health-record and telehealth systems. Approved additional-state licensing reimbursement , ordinarily paid at an additional $40 per completed eligible encounter until the agreed expense cap is reached. Licensing reimbursement is limited expense support, not a permanent increase in clinical compensation. The practice handles billing and bears ordinary collection risk as described in the contractor agreement. Clinicians remain responsible for their ordinary professional tools, computer, travel, parking, taxes, and independent-business expenses unless otherwise agreed. Meaningful Primary Care, Not Just Isolated Visits Our clinical model emphasizes thoughtful evaluation, modern evidence-based treatment, and continuity. You may provide chronic disease management, preventive care, annual wellness visits, and appropriate episodic acute care. Areas of interest include cardiometabolic health, obesity medicine, sleep apnea and insomnia, primary-care mental health, migraine, common dermatologic conditions, and men's and women's health. The in-person setting supports physical examination and locally accessible care, while telehealth offers another way to maintain continuity when clinically appropriate. This is a shared-space outpatient model, not an emergency department or unrestricted procedural clinic. Procedures, injections, phlebotomy, dispensing, and other services requiring additional equipment or site authorization are outside the default scope unless separately approved and supported. Reasonable Visit and Paperwork Boundaries Clinicians may generally structure an appointment around two to three stable chronic problems, one new problem, or one unstable chronic problem Additional nonurgent concerns may be scheduled separately when they cannot be adequately addressed within the visit. These are workflow and patient-safety boundaries, not coding criteria. Urgent concerns must still be assessed or appropriately escalated. Requests involving FMLA, disability certification, accommodations, extensive prior-authorization appeals, or substantial care coordination may require a separate appointment when additional clinical evaluation is necessary. Material nonroutine administrative work requires an appropriate compensation arrangement or independently qualifying service. Routine documentation and reasonable encounter-related follow-up remain part of clinical responsibility. Forms are not automatically billable simply because they require time. No Controlled-Substance Prescribing Requirement There is no expectation to prescribe controlled substances or obtain multiple DEA registrations . Opioid prescribing and chronic opioid management, along with ongoing management of chronic daily benzodiazepine or Z-drug therapy, are outside the ordinary scope of this position.
MAT/MOUD
services require a separate written program agreement. Other controlled-substance prescribing is optional and requires appropriate authority, credentialing, coverage, practice approval, and independent clinical judgment. Who Will Thrive Here? Preferred qualifications include:
Active, unrestricted NP licensure and national certification appropriate to adult primary care. At least 18 months of primary-care or urgent-care experience preferred , with demonstrated readiness considered individually. Confidence managing a broad range of common presentations, coupled with sound judgment about consultation, referral, and escalation. Comfort working in a lean shared-space setting, maintaining accurate documentation, and managing accepted patients' results, messages, and follow-up. Additional state licenses are valuable for optional telehealth work. Evening and weekend availability is particularly welcome when the premises are available, although never required. These periods may offer stronger demand and opportunities for eligible after-hours service compensation. Professional Growth for Clinicians Who Want to Build We welcome clinicians who want to contribute beyond their own appointment schedule. As the practice expands, opportunities may arise to develop focused clinical programs, improve care pathways, contribute to quality initiatives, support other clinicians, or take on clinical and organizational leadership responsibilities. Interests in sleep medicine, cardiometabolic care, obesity medicine, primary-care mental health, and other complementary services are especially relevant. Clinical excellence, initiative, and reliable execution matter here. We are interested in people who identify a problem, propose a thoughtful solution, and help carry it through. Program-development and leadership work would be defined through separately agreed responsibilities and compensation. Alternative clinical fee arrangements may also be considered for objectively different services, expertise, or coverage commitments and must be established in writing before the work begins. Independent-Contractor Structure This is a nonexclusive 1099 independent-contractor opportunity , not a salaried employment position. There is no guaranteed minimum income, patient volume, or employee-benefit package. The role is intended for clinicians seeking to develop an in-person practice with optional telehealth, while retaining control over the availability they offer and the appointment lengths they select.
