Join Arches Medical as a Nurse Practitioner or Physician Assistant. We have multiple locations throughout Rhode Island. Join the frontlines of today's healthcare transformation Overview Arches Medical Partners is a physician-owned and operated primary care group focused on restoring the valued relationship between patients and their PCPs and improving access to care in our communities. We are seeking an NP/PA to manage an adult patient panel, delivering personalized chronic care, acute care, and preventative services. In this role, you'll work in a practice that empowers physicians and advance practice clinicians with decision-making authority and places patient care at the center, emphasizing high-quality outcomes and holistic value-based care.
Responsibilities:
Primary Care Management:
Provide comprehensive care, diagnose, treat, and develop patient-centered treatment plans as patients' PCP of record.
Preventive & Wellness Care:
Conduct health screenings, vaccinations, and educate on preventive strategies.
Value-Based Care Delivery:
Use data to identify care gaps, support population health, and improve outcomes.
Care Coordination:
Collaborate with an interdisciplinary team to deliver integrated, high-quality care.
Documentation & Compliance:
Maintain timely accurate electronic health records (EHR) documentation.
Practice Innovation & Performance Accountability:
Drive practice transformation, embrace new tools, and achieve clinical and financial goals.
Qualifications:
FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA Current Advanced Practice license. Strong skills in adult primary care and chronic disease management. Experience in Family Medicine, Internal Medicine, Gerontology, Palliative Care or home care Experience or interest in value-based care and practice transformation preferred. Excellent communication and interpersonal skills.
