Job Description

About the Clinic The Community Medical Clinic of Kershaw County is a community-based free medical clinic dedicated to providing accessible, high-quality healthcare to underserved populations regardless of their ability to pay. Our mission is to promote health equity and wellness through compassion, education, and collaboration. Current service area is Kershaw County and portions of Lancaster County. Position Summary The Nurse Practitioner (NP) will provide primary and preventive healthcare services to patients of all ages in a free clinic setting. This role emphasizes patient-centered care, health education, disease prevention, and chronic disease management. The NP will work collaboratively with Physicians, Nurse Practitioners, nurses, and volunteers to ensure comprehensive and culturally sensitive care. Work settings may include main clinic, satellite locations, and school-based health centers. Clinic is open Monday through Thursday 8:00 am - 5:00 pm. Key Responsibilities Conduct comprehensive patient assessments, including physical exams, medical histories, and diagnostic evaluations. Diagnose and treat acute and chronic illnesses consistent with established clinical guidelines. Prescribe medications and therapies within the scope of practice and state regulations. Order and interpret laboratory tests and imaging studies as appropriate. Develop individualized care plans that consider patients' social and economic contexts. Provide health education, counseling, and preventive care guidance. Collaborate with multidisciplinary team members and community partners to coordinate referrals and follow-up care. Maintain accurate and timely documentation in EPIC, electronic health record (EHR). Participate in quality improvement initiatives and ongoing clinic development efforts. Support clinic outreach, health fairs, and patient education sessions as needed.
Qualifications Education:
Master's or Doctorate degree in Nursing from an accredited program.
Licensure:
Current Nurse Practitioner license and prescriptive authority; national certification (FNP, ANP)..
Experience:
Minimum of 2 years of clinical experience preferred; experience in community health, primary care, or underserved populations strongly desired.
Skills:
Strong clinical assessment and decision-making abilities. Excellent communication and interpersonal skills. Culturally competent, empathetic, and adaptable. Ability to work collaboratively in a team-based environment. Working Conditions Fast-paced, collaborative environment in a community clinic setting.
