Job Description

Job Overview We are seeking a dynamic and compassionate Wound Care Nurse Practitioner to join our multidisciplinary healthcare team. In this vital role, you will provide expert wound assessment, management, and treatment to promote healing and improve patient outcomes. Your expertise will support a diverse patient population with high acuity, including those in acute care, outpatient clinics, long-term care facilities, and inpatient hospital setting. This position offers an exciting opportunity to utilize advanced clinical skills, collaborate with specialists, and make a meaningful difference in patients' lives through innovative wound care practices. Duties Conduct comprehensive wound assessments, including evaluating wound etiology, size, depth, and infection status using clinical examination and diagnostic tools such as sonography and laboratory testing. Develop personalized wound management plans incorporating advanced treatments like dermal fillers, suturing techniques, and sterile processing protocols. Coordinate with interdisciplinary teams to ensure optimal patient care, discharge planning, and utilization review across inpatient, outpatient, and post-acute settings. Educate patients and families on wound care techniques, infection control measures, nutrition's role in healing, and disease management strategies. Document all patient interactions accurately within
EPIC EMR
systems while adhering to medical terminology standards like
ICD-10/ICD-9
coding. Qualifications Current Nurse Practitioner licensure with certification in wound care or related specialties. Proven experience working with complex patient populations including those in critical care units (ICU), trauma centers (Level I/II), or emergency medicine environments. Strong knowledge of anatomy, physiology, and diagnostic evaluation techniques relevant to wound management; familiarity with telehealth systems is advantageous. Hands-on experience with medical office procedures such as suturing, sharp debridement, and sterile technique. Proficiency in EMR/EHR platforms like Epic; understanding of CPT coding for billing purposes is preferred. Ability to perform physical examinations, vital signs assessment, and patient care planning effectively across diverse settings. Excellent communication skills with the capacity to triage urgent cases swiftly while demonstrating compassion for patients with complex needs like dementia or memory care requirements. $120,000.00 - $145,000.00 per year
