Nurse Practitioner Job Description Overview We are seeking a dedicated and compassionate Nurse Practitioner to join our family practice team. The ideal candidate will provide high-quality patient care in various settings, including primary care, inpatient facilities, and assisted living environments. This role requires a strong understanding of medical practices, patient management, and the ability to work collaboratively with other healthcare professionals. The Nurse Practitioner will be responsible for diagnosing and treating a wide range of health conditions while ensuring the highest standards of care. Duties Conduct comprehensive assessments of patients' health status through physical examinations and medical histories. Diagnose and manage acute and chronic illnesses, utilizing ICD-10 coding for accurate documentation. Develop and implement individualized treatment plans, including prescribing medications and therapies. Provide education to patients and families regarding health conditions, treatment options, and preventive care strategies. Perform medical procedures such as botulinum toxin treatment, catheter care, and other necessary interventions. Collaborate with interdisciplinary teams to coordinate patient care across various settings, including inpatient facilities and assisted living environments. Utilize medical imaging as needed to assist in diagnosis and treatment planning. Advocate for patients with disabilities by addressing their unique healthcare needs effectively. Half day Saturday hours will be required, 2 to 3 Saturdays a month. Qualifications Master's degree in Nursing or a related field with a valid Nurse Practitioner license. Proven experience in primary care or inpatient settings is preferred. Familiarity with medical coding systems such as ICD-10 is essential. Strong clinical skills with the ability to perform various medical procedures competently. Excellent communication skills to interact effectively with patients, families, and healthcare teams. Knowledge of working with individuals with disabilities and understanding their specific health requirements is a plus.
